For doctors, investors and healthcare groups turning a shell or an old office into a clinic, dental practice or pharmacy. We line up the health-authority layout, the building and fire approvals and the fit-out so the facility is ready for its licensing inspection.
A clinic fit-out has two finish lines. The building has to be approved and completed like any commercial fit-out, and the facility then has to pass the health authority's inspection before a single patient can be seen. Plan only for the first, and you can end up with a finished clinic that cannot open.
This page looks at the project: choosing the unit, designing the layout, getting the building, fire and services approvals, and arriving at the health inspection with everything in place. We are a private consultancy. The health authority, the building authority and Civil Defence each decide on their own approvals. We coordinate the drawings and applications so they agree with each other.
For facilities licensed by the Dubai Health Authority, licensing is a two-step process. New Facility License gives an inactive licence, which DHA's licensing manual says is valid for one year. Activate Facility License follows a successful DHA inspection. The facility layout is part of the first step.
The fit-out permit or modification approval from the authority for the location, the landlord's NOC, Civil Defence approval and any DEWA or other service approvals. It ends with completion and the fire safety certificate.
The tracks meet twice. First, the layout DHA reviews must be the same layout the building authority approves. Second, the activation inspection expects the building side to be finished: DHA's manual lists a copy of the approved engineering layout and a copy of the Civil Defence certificate among the items the facility should have ready.
The one-year clock: under DHA's licensing manual, failing to complete the licensing requirements within one year of the inactive licence leads to cancellation of the process and a fresh application. A slow fit-out approval can therefore cost you the health licence application.
Most clinics and pharmacies in Dubai are licensed by DHA, whatever building authority covers the unit. The main exception is Dubai Healthcare City, a free zone with its own regulator (Dubai Healthcare City Authority – Regulation) that licenses facilities and professionals inside the zone.

Many problems are fixed, or created, when the lease is signed. Before committing, check:
DHA's licensing manual says the facility layout or floor plan must be designed and approved by prequalified health facility design consultants, with room and bed counts as per the DHA Health Facility Guidelines. DHA publishes the list of prequalified consultants in two categories: Category 1 for the smallest and least complex facilities, and Category 2 for all levels including hospitals and surgical facilities. For pharmacies, DHA's guidelines ask for schematic drawings in AutoCAD format with measurements and labels for each room or area.
The same plan then has to work for the building authority and Civil Defence. That is where conflicts appear: a DHA-compliant room layout that creates a dead-end corridor, or a fire-rated wall the health planner did not expect. We coordinate the health facility planner, the fit-out designer and the MEP and fire engineers so there is one drawing set, not two that disagree.

Licensed facilities are not free to rebuild. DHA's licensing manual lists "renovate/change layout" and "expand facility building" among the changes that need an amended facility licence, and a move needs the change-of-location service with an updated floor plan and a field inspection. DHA's pharmacy guidelines also require prior approval for adding an extension or annex. On the building side, the works need a fit-out or modification permit as usual. Plan both together so the clinic is not left operating a layout that only one authority has approved.
It is risky. If DHA requires changes to rooms or counts, built work may have to be redone. We usually aim for the DHA layout and the building permit to be settled before construction starts.
DHA's licensing manual says the layout must be designed and approved by a prequalified health facility design consultant from DHA's published list. The fit-out drawings for the building authority are then prepared to match it.
DHA's pharmacy guidelines set 30 m² for a community or retail pharmacy and 25 m² for an ambulatory pharmacy within an outpatient clinic, with a ceiling height of at least 2.70 m.
DHA's manual lists a copy of the Civil Defence certificate among the items the facility should have ready for the activation inspection, so the fire approvals and completion should come first.
DHA has a change-of-location service for licensed facilities, with an updated floor plan and a site inspection. The new unit still needs its own fit-out and fire approvals.
